Your account number can be found on documentation sent to you by UI, or by logging into the MyUI Employer system. Please enter the 9-digit number excluding any dashes or periods.

Your FEIN can be found in tax documents sent to you by the IRS. Please enter the 9-digit number excluding any dashes or periods.

Claimant Information

This a unique eight-digit claimant identifier used to locate claimants in the Unemployment Insurance (UI) system. Claimants can find their id number in their MyUI+ account. Employers can find their employees' claimant id numbers on paperwork sent to them from the Colorado Department of Labor & Employment.
Your claimant id can be found on your home page in MyUI+, next to your name, as seen in the SAMPLE screenshot below:

Remember: If you provide an incorrect Claimant ID, your file will not be received by UI.

The document number (such as 'UIQC-5) can be found in the bottom left corner of the document.

This field converts the picklist value of the 'what type of file are you uploading?' question into a numerical value for use in the salesforce connector.
Note: If you are submitting a UITR-1 and a UITR-1a together, please select the 'UITR-1' option. Otherwise, if you are submitting an UITR-1a by itself, please continue.
Remember: Although you can submit the paperwork after the due date, failures to report, disclose, or provide information when directed or to complete the Benefit Accuracy Measurement questionnaire by the due date may result in a delay or denial of benefits.

Incarceration Investigation
Use this form if the Unemployment Insurance division has contacted you regarding time you may have spent incarcerated. Refer to the email you received for details and your case number.
Use this form if our Benefit Payment Control team has contacted you via email in regard to an individual who may have been incarcerated at your facility. Each file uploaded will be attached to one specific claimant's case, so please include information about the claimant referenced in the email only, and no other individuals.

The case number will be in the following format: BPC-123456-MM-YYYY. If you are unable to locate the case number on the email you received, contact the Benefit Payment Control team directly at 303-318-9035 (Denver-metro area) or 1-877-464-4622 (outside Denver-metro area)
IMPORTANT: If you mistype or enter an incorrect case number, the Unemployment Insurance division will not be able to receive your submission.

File Upload


Supported file extensions include doc, docx, xls, xlsx, ppt, pptx, pdf, odt, odt, ods, odp, bmp, gif, jpg, jpeg, png, tif, qtf, and txt.
If possible, combining multiple files into one single file will make it easier for UI to review and process your documents (for instance, sending a single combined .pdf file instead of several individual .pdf files).

In order to avoid connection errors, this form only allows users to submit up to five files at a time (see 'Click here to add an additional file', below). However, you may return and use this form again if you have more than five files to submit.
